Job Description
- Serves as a back office support to Parent Company that is based in San Diego, California USA, particularly catering to Home Health and Hospice care.
- Acts a liaison officer between the Parent Company in relation to patient care and other home health/hospice related services.
- Monitor patient’s overall status from referral to admission and until the patient is discharged from the home health or hospice services.
- Quality audits of patient’s assessments and charting.
- Ensure accurate and timely Home Health and Hospice documentation.
- Provide technical support to counterparts specifically in using electronic medical records and accurate clinical documentation.
